Abstract
The older population is persons aged 65 and older. Many older adults live healthy and active lives while others have one or more illnesses or comorbidities that make it difficult for them to live alone. Older adults aged sixty-five and older who have one or more comorbidities often require assistance to remain in their homes for as long as possible. Older adults who do not have this needed assistance often have frequent falls, make frequent trips to the emergency room, have frequent hospital admissions, or end up in a nursing home.
Product one of the Capstone Presentation is the systematic literature review that discusses the barriers to access needed care in the home older adults face to age in place. There are many barriers that may prohibit an older adult from aging in place, but this product focuses specifically on caregivers. Product two of the Capstone presentation is the conceptual paper. This paper discusses the current solutions and why they do not currently work for accessing caregivers. This chapter will relate Ecological Systems Theory to the older adult and how the five subsystems: microsystems, mesosystems, macrosystems, exosystems, and chronosystems, work together to determine the quality of life for an older adult to age in place. Product three is the Practice Application Paper and will look at how changing Medicare policy to include a benefit for eligible older adults to acquire the assistance they need to age in place.
This presentation will discuss improved quality of life and savings to Medicare by incorporating this benefit. In conclusion, this presentation will discuss the incorporation of a community brief to educate other social workers and stakeholders on this policy change and the need for this benefit.
