Abstract

Behavioral health challenges in rural communities continue to place significant strain on healthcare systems, particularly through repeated reliance on emergency departments for crisis care. Nelson County, Kentucky, reflects this pattern, with higher reported rates of poor mental health and limited access to behavioral health providers. This paper proposes the Beacon Behavioral Health Response Program, a community-based, multidisciplinary crisis intervention model designed to improve access to timely care and strengthen continuity of support. The program combines mobile crisis response—delivered by a licensed clinical social worker and a peer support specialist—with a technology-enabled component using the Luma AI-supported mental health platform. Together, these elements aim to provide rapid assessment, de-escalation, and connection to follow-up services while offering ongoing support between crisis events. The program is supported by existing evidence on mobile crisis teams, peer support integration, and digital mental health interventions, particularly in underserved and rural settings. A structured management, financing, and sustainability plan outlines implementation through local partnerships and diversified funding sources, including Medicaid reimbursement and grant opportunities. A 12-month evaluation will assess both process and outcome measures, including response times, follow-up engagement, and changes in emergency department utilization. By shifting crisis care toward a more responsive, community-based model, the program aims to reduce repeat crises and improve long-term mental health outcomes.
