Abstract

This paper presents a comprehensive evaluation plan for the Bourbon County Physical Activity Improvement Program, as prompted in the University of Kentucky's 2025 MPH-Health Management and Policy Capstone Case Study. The case study outlines a program developed in response to high obesity and inactivity rates in Bourbon County, Kentucky. Utilizing the CDC's 6-step program evaluation framework, the plan outlines engagement of a diverse group of stakeholders to assess the program's effectiveness in increasing physical activity among residents. The evaluation focuses on both process and outcome measures, examining the implementation of exercise facilities, community events, and an app designed to track physical activity. Using example findings indicating a modest increase in physical activity, particularly in urban areas, the need for tailored strategies to enhance engagement in rural communities is explored. Recommendations include expanding recreational opportunities and enhancing app features to sustain user interest. The study underscores the importance of stakeholder engagement and continuous feedback to ensure the program's success and potential replication in other counties.
